Patient Information

If you would like to join the practice you will need to provide proof of address, such as a utility bill and your NHS number when you register.

You will be given a registration form (or you can download here) a short form and a questionnaire to complete.

Once the paperwork has been completed and returned to the practice you will need to book an appointment with our Health Care Assistant for a New Patient Check.

If you are prescribed regular medication, you will also need to book an appointment with our clinical pharmacist.

For children under 16 a parent or guardian can register on their behalf. Please provide the child’s NHS number and sign the registration form as parent or guardian.

As a patient, you have the right to express preference as to which GP you would like to see, however, please bear in mind it may not always be possible to see your preferred GP.
