Fit Note / Sick Note

How to Request a Fit Note

  • Submit an online Anima request Explaining: why you need the fit note, when you became unwell, how long you think you'll be unable to work (if known).

  • call the surgery on 01622 745 585 if you can't use the online service. Reception can submit an Anima request on your behalf, and the clinical team will decide whether they can issue the fit note.

​💡You must self certify first for the first 7 days of being off work. Once you pass this time period you can then request a Fit Note / Sick Note.
