Travel Advice
❗Please note the Practice can only provide the following travel vaccinations, which are currently available free to NHS Patients: –Hepatitis A, Cholera , Typhoid , Polio (given as a combined diphtheria/tetanus/polio jab)
To arrange your travel vaccinations we firstly need you to complete & return our Travel Risk Assessment Form at least 6-8 weeks before your departure.
Please email this completed form to mydoctor@nhs.net with the subject heading “Travel Vaccination” including your departure date in the title. One of our Nursing Team will then review the form then contact you to discuss and to arrange an appointment.
